Obverse: Within an oak wreath a foul anchor with a crown above. Reverse: inscription within a pearl border. Legend: "FOR LONG SERVICE AND GOOD CONDUCT". Inscription incuse: "W. Patchet, R.M. H.M.S. Blonde 22 years". Fitted with a bar and dark blue ribbon.
